Summa Health System focuses on reinvestments to support the community, specifically through volunteerism, sponsorships and support of community initiatives through Summa. To submit a sponsorship request, please complete a Sponsorship Request Form and attach any sponsorship request materials (e.g. letter, flyer, packet of information, etc).

The Community Reinvestment Board reviews sponsorship requests on a monthly basis. Please submit it at least 60 days in advance.

If awarded funding, your organization must complete and return the required Follow-up Documentation Form and attach a letter which contains information on the amount of funds received and information on any amount that is tax deductible.
