Maintaining independence is an important part of the aging process. The Summa Center for Senior Health specializes in coordinating every aspect of an older adult’s care in order to preserve independence and help you and your family cope with the medical, emotional and social problems commonly associated with aging. Geriatricians, geriatric-certified advanced practice nurses and social workers who have special training, testing and certification in the care of older adults meet with each patient and their family to carefully develop individualized care plans addressing every aspect of well-being. The Center’s excellent staff can offer guidance managing many common problems older adults face such as:
- Declining ability to carry out every day tasks such as bathing
- Medication problems
- Chronic and disabling illness
- Confusion and memory loss
- Unsteadiness and falling
- Depression
- Incontinence
- Decisions about living arrangements
- Caregiver stress or family problems
Your personal physician can refer you and your family to the center. The center's staff will help you arrange an appointment for a comprehensive health evaluation by a team specially trained in the care of older adults. How Much Does it Cost?
Medicare reimburses 80 percent of the cost. Depending on your insurance situation, a co-payment may apply or supplemental insurance may pay the rest. Most HMOs pay 100 percent of the cost when you have an authorized referral from your primary care physician.
You and your family are encouraged to talk about financial matters or concerns with the center's social worker.
