Summa’s HomeCare Program serves individuals who meet specific criteria for skilled nursing or other services. A Summa HomeCare nurse works with a patient’s primary care physician to arrange for medical services which may include:
- Cardiology
- Oncology
- General medical services
- Orthopaedics
- Geriatric assessments
- Palliative care and hospice services
- Infusion therapy
- Patient education
- Maternal care
- Physical therapy
- Medication management
- Speech therapy
- Occupational therapy
- Wound care
Summa HomeCare nurses deliver care to those recovering from surgery, an injury or a recent hospitalization in the comfort of the patient’s home. It is designed to support patients during the transition from hospital to home – and back to their primary care physician.
The program’s goal is to prevent and/or reduce unnecessary hospital readmissions.
HomeCare nurses work closely with each physician to arrange for home care services designed to meet each patient’s unique needs. HomeCare services are available seven days a week, 24 hours a day, including holidays.
