Summa's Care Coordinators
A care coordinator provides individualized assistance offered to patients, their families, and caregivers to help overcome barriers to care, whether through the health care system, or the environment, and facilitate timely access to quality medical and psychosocial care through all phases of the cancer experience.
Our care coordinators provide a warm relationship of support and coordination of care for the patient and family. Care coordinators also provide education of the disease process and treatments. In addition, the coordinator helps to facilitate access to supportive, financial, and psychosocial services. Our care coordinators along with a collaborative multidisciplinary team work closely to provide efficient access to the health care system.
