Bridge to Home

Bridge to Home is a program for patients with SummaCare Secure Insurance who are diagnosed with congestive heart failure, COPD, Afib, pnemonia, chest pain, shortness of breath, small bowel obstruction and gastrointestinal bleeding - conditions where patients can present with common complications that could cause a readmittance in a short period of time. A nurse establishes contact with patients - both in the hospital and at home. 

Patients are taught how to talk to - and ask questions of - their primary care physicians, and navigate through issues, such as how to get a referral that can keep them out of a hospital.
