Outpatient Diabetes Education and Support
The Summa Diabetes Education and Self Management Program offers a multidisciplinary approach to diabetes education, combining the patient, family, physician, certified diabetes educator, nurse educator and dietitian. Together, the team helps the patient learn general information about diabetes and its maintenance through discussions about medications, nutrition, personal care, and prevention of complications, exercise and more.
Summa’s diabetes education and self-management programs focus on the AADE7 Self Care Behaviors essential for improving health and quality of life, which include:
- Healthy eating
- Being active
- Problem solving
- Healthy coping
- Reducing risks
Our education and support program offers:
- Group classes (patients may be accompanied by one adult)
- Individual sessions
- Refresher classes (yearly update)
- Gestational diabetes sessions
- Medical nutrition therapy (diabetes and other related diseases)
- Free community education seminars for those with diabetes and their family members
Class content. Summa diabetes education classes cover all content recommended by the American Diabetes Association (ADA), including:
- Diabetes disease process
- Physical activity
- Monitoring/using measurement results
- Acute and chronic complications
- Goal setting and problem solving
- Psychosocial adjustment
- Preconception care, pregnancy and Gestational Diabetes (when appropriate)
Each participant in Summa’s comprehensive program receives an initial assessment interview by a diabetes educator to determine the educational needs. After the assessment, participants will attend either individual or group sessions.
After the series of educational sessions, participants attend a follow-up appointment with dietitians. Follow-up calls from the staff gauge patient progress.
Further education and programs are offered throughout the year, including cooking demonstrations and speaker presentations at support group sessions like the following:

The Diabetic Support Group is an educational program covering various topics concerning diabetes. Such topics may include proper nutrition, medications and other concerns among the group. Group members can ask questions, learn helpful information and meet others with diabetes.
Meeting Dates: *Last Saturday of each month, 8:30 a.m.-9:15 a.m. Free, no registration required.

The Diabetes CARE programs are free educational sessions covering various topics concerning diabetes for patients and their friends and family. We have speakers from the medical community including physicians, dietitians, social workers, as well as motivational speakers.
