At one time, many doctors made house calls. Patients appreciated the convenience of home healthcare and found great comfort and compassion in receiving healthcare services in the privacy of their homes.
House calls are particularly helpful for patients who cannot leave their homes because of chronic, severe illnesses such as diabetes, hypertension, pneumonia, stroke or chronic heart failure, or those who have physical disadvantages such as difficulty walking or fear of falling. To better meet these patients’ needs, SummaCare and Summa Health System are pleased to offer house call services to traditional Medicare patients in Summit County, Wadsworth and Sharon Center, as well SummaCare Secure patients in Medina, Portage, Stark and Summit Counties.
A dedicated team led by a geriatrician will provide clinical care to SummaCare Secure and traditional Medicare (non-HMO) patients. Following an initial house call visit by a geriatrician, other members of the team, including a social worker and nurse practitioner, will be involved in taking care of the patient.
While this program is open to both SummaCare Secure and traditional Medicare patients, there are additional benefits available to patients enrolled in the SummaCare Secure health plan. A SummaCare Case Manager is available to help individuals access additional healthcare resources, and a licensed practicing pharmacist will review medications and help identify any potential issues such as side effects.
The House Calls Program is also affordable. There are no out-of-pocket costs for house call visits for SummaCare Secure patients. However, traditional Medicare patients are responsible for normal co-pays and deductibles, just as they are when they go in for a visit with their current doctor.
