How to Access MyChart with your results within

Go to summahealth.org/mychart. Click on the via secure
login button on the right side of the page. If you received messaging View
a MyChart activation code, click SIGN UP NOW and Review progress
enter your access code. You will be prompted to set up wellness notes for
your unique MyChart user name and password. If you plans outpatient
already have a user name and password, enter them and visits
click SIGN IN.
