Support and Follow-Up After Breast Cancer 

The Summa Health Survivorship Clinic invites any breast cancer survivor to find support, guidance and feel confident in your care at any point of your breast cancer journey. The Survivorship Clinic offers individualized evidence-based guidelines for current survivorship care that can improve your quality of life, well-being and longevity. Our specialty care breast providers are experts at recognizing and addressing the special needs of a newly diagnosed survivor and if they are adjusting to their “new normal.” Our team is available to provide ongoing long-term survivorship care to all breast cancer survivors. 

Clinic Visits 

As a survivor, you will alternate visits between your surgeon and the nurse practitioner who specializes in breast health and breast cancer. Surgeon Visits Your surgeon will continue to oversee your care, complete exams and will always be available to you. If you are new to Summa Health and our survivorship clinic, we welcome you to visit our clinic. We are happy to coordinate your survivorship initial visit with one of our specialty breast care providers. 

Nurse Practitioner Visits 

When you see the nurse practitioner, you will have a longer appointment time so that your health can be assessed. You will receive a physical exam and all of your questions and needs as a survivor can be discussed. At this visit, the nurse practitioner will discuss with you important recommendations for healthy lifestyle behaviors, understanding, recognizing and managing treatment side-effects, and what signs to report that may be related to a recurrence or second cancer.

• Weight management 
• Exercise 
• Skin care 
• Healthy lifestyle 
• Alcohol, tobacco and caffeine use 

At a survivorship visit, you will work with your breast specialist and using shared decision making, develop a plan for ongoing breast cancer surveillance and survivorship care.
