Cost is just $40 out of pocket, without billing insurance. Walk-ins ONLY. Please bring your school's sports physical forms.

Extended Hours to Better Serve You
Sports Physicals are offered at all Summa Health Urgent Care Centers during the following hours:
- Monday - Friday, 9 a.m. to 6:30 p.m
- Saturday and Sunday: 9 a.m. 4:30 p.m.
- Call for holiday hours.
*Elementary and High School Students, age 18 or younger
