Geriatric Risk Assessment

As we age, we'd like to remain in good health, stay independent and live a fulfilling life. However, with age often comes the potential for complex medical issues. Whether you are a senior or a caregiver,  there comes a time when you may need to seek the help of a doctor who specializes in the treatment of older adults. Answer the questions below regarding yourself or a loved one to help you find out. Upon completion, share the test results with your doctor.

Geriatric Risk Assessment

Is your loved one showing signs of cognitive impairment (forgetfulness, confusion, mood swings, disorientation)?
Do they have trouble walking or have falls?
Were they hospitalized within the past three months?
Do they take more than five medications daily?
Do they rate their health as worse than others?
Does their primary care provider recommend they be seen by a geriatrician due to the complexities of caring for an older adult?
