For patients with chronic, severe illnesses, we offer health services for some older patients in the convenience of their homes.

Summa House Calls

At one time, many doctors made house calls. Patients appreciated the convenience and found great comfort and compassion in receiving healthcare services in the privacy of their homes.

House calls are particularly helpful for patients who cannot leave their homes because of chronic, severe illnesses such as: 

  • Diabetes
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Stroke
  • Chronic heart failure
  • Individuals who have memory loss that limits the ability to get out of the house for medical appointments
 

To better meet these patients’ needs, SummaCare and Summa Health are pleased to offer this time-honored service.

Your care team is led by a Nurse Practitioner (NP) who specializes in the care of older adults and partners with your doctor. Your geriatrician is a medical doctor who has been specially trained in the aging process and focuses on treating older adults. Your geriatrician and NP make home visits regularly. 

Depending upon your condition, you may also see other healthcare providers, such as a social worker, pharmacist and nurse case manager. Your team meets regularly to discuss your care. 

The House Calls Program also is affordable. There are no out-of-pocket costs for house calls for SummaCare Secure patients. However, traditional Medicare patients are responsible for normal co-pays and deductibles, just as they are when they go in for a visit with their current doctor.
