Sports Physicals

Keep Your Student-Athlete Healthy, Safe and Ready to Play

Before your child can hit the field, court or track, most schools and athletic organizations require a pre-participation physical exam (PPE), commonly known as a sports physical. This important screening helps detect any health concerns that may affect safe athletic performance.

Summa Health sports physicals include:

  • Medical history review and physical exam
  • Flexibility, strength and joint function checks
  • Blood pressure, pulse, height and weight measurements

Required Forms

Bring your school or league’s physical form to the physical. The athlete’s health history should be filled out in advance. Our providers can typically complete the remainder of the form during the visit.

Cost

Cost is $40 out of pocket, without billing insurance, for elementary and high school students ages 18 or younger.

Walk-Ins Welcome

Walk-in visits are available Monday- Friday from 8:00 a.m. to 3:30 p.m. at the following orthopedic locations.

Sports physicals can also be scheduled with a Summa Health sports medicine provider at our orthopedic locations.

Visit an Urgent Care Location

Sports physicals are also available at all Summa Health Urgent Care locations. Those locations include Fairlawn, Green, Tallmadge and Stow-Kent. Our hours are:

  • Monday through Friday: 9:00 a.m. to 6:30 p.m.   
  • Saturday and Sunday: 9:00 a.m. to 4:30 p.m.
  • Call for holiday hours.
