Discharge & Continuing Care

Your care doesn't end when you leave the hospital. As you prepare for discharge, our team will help you understand your next steps and connect you with the resources and support services you may need for recovery and ongoing health.

Your Discharge

Before you leave the hospital, your physician must write a discharge order. Your care team will review any medications, follow-up appointments and home care instructions with you to help ensure a smooth transition home. When it is time for discharge, a hospital employee will escort you by wheelchair to the designated discharge area.

After Your Stay

Patient Experience Survey

Your feedback helps us improve. After your hospital stay, you may receive a patient satisfaction survey by mail. We encourage you to share your experience and let us know how we can better serve our patients and families.
 

Home Care
