Your physician must write a discharge order before you can leave the hospital. It is important you remember to check with your physician or nurse for any special medications or home care instructions prior to your departure from the hospital. At the time of your discharge, a hospital employee will escort you in a wheelchair to the discharge area.
Summa Health System wants you to have the most positive experience possible while you are a patient in one of our hospitals or facilities; therefore, you may receive a satisfaction survey which will be mailed to your home after you leave the hospital. Please provide us with your comments by completing and mailing the survey.
If you do not receive a survey and would like to give us your feedback, please call Summa Connections 800.237.8662 and one will be mailed to you.
