Summa Health recognizes that health disparities exist in our community. And we are committed to transforming the care you receive.
Health disparities are preventable differences in disease that impact how frequently a disease affects a group, how many people get sick or how often the disease causes death. Health disparities can be related to a number of factors. The Summa Health Equity Center’s goal is to provide you with outstanding, quality care and identify, reduce and eventually eliminate health disparities to create health equity for all.
The Summa Health Equity Center provides access to multiple programs, designed to provide the tools you need to sustain a healthy lifestyle. This community-based care will empower you to play a proactive role in your healthcare, through chronic disease management, nutrition, exercise and healthy lifestyle education services.

Care Coordination
From ongoing medical concerns to social drivers of health – the Equity Center facilitates continuity of care across settings and among providers.
By being aware of your individual needs and preferences, the Summa Health Equity Center care coordination team can help communicate them at the right time to the right people, all in an effort to improve your care quality and satisfaction, as well as the effective and efficient use of healthcare resources. Rely on the Summa Health Equity Center to serve as your medical hub, where all your healthcare needs are managed by one dedicated team.

The Equity Center recognizes the benefits of wellness and prevention as a part of your overall good health. Our collaborative team will work closely with you to develop safe and healthy exercise and nutrition plans that meet your personal health and fitness goals, all to help you get healthy – and stay healthy.
