Our team of clinical pharmacists is trained in home infusion and is available 24 hours a day. Each care team member is well-equipped to assess, educate and care for infusion patients.
In addition to providing education and care, we closely monitor the patient's clinical status, dosage frequency and treatment efficacy. The expertise and extensive experience of the care team allow them to identify, communicate and respond to any changes in the patient's status related to the applied therapy. Our team offers collaborative hospital discharge planning, clinical consulting and therapeutic monitoring of drug therapies when appropriate, including drug interactions, laboratory reports and pharmacokinetics.
