Summa Health’s Post-Acute Care Division provides specialized medical and rehabilitative services to patients after they have been discharged from a hospital or acute care setting. It is a phase of care that focuses on helping patients recover after a severe illness, injury or surgery. Post-acute care is designed to bridge the gap between acute care in a hospital and returning to the patient's home or community.

Post-Acute Care Services

Home Care
Services are provided in the home and may include medical, psychosocial and symptom assessments, wound care, disease education and management, physical therapy, occupational therapy or speech therapy.

Hospice Care
A holistic approach to caring for patients who are terminally ill and wish to focus on their quality of life. This involves a team of trained professionals, available 24 hours a day, who provide medical attention, symptom management and emotional and spiritual support.

Home Infusion Therapy
Involves the intravenous or subcutaneous administration of drugs or biologicals to an individual at home.

Inpatient Rehab Facility (IRF)
A hospital that specializes in treating patients who require a combination of hospital-level care and intensive rehabilitation in order to regain the level of function needed to return home.

Long-Term Acute Care (LTAC)
A specialty-care hospital designed for patients with serious medical problems that require intense, special treatment for an extended period of time and require more medical care than a skilled nursing facility, nursing home or acute rehabilitation.

Skilled Nursing Facility (SNF)
Provide short-term care for patients recovering from an illness or injury who require skilled nursing and/or skilled rehabilitation services.
