Advancing Age-Friendly Care Through Medication Optimization

Please join staff members of Summa Health Complex Care Institute and other medical professionals for a day of discussion and inquiry. The symposium will focus on medication safety, appropriateness and person-centered prescribing for older adults by applying the Medication M of the 4Ms framework across clinical settings, disciplines and transitions of care. The event will include lectures, panel discussions and interaction with community partners.

This one-day educational event is designed for multidisciplinary care team members including physicians, advanced practice providers, nurses, social workers, pharmacists, therapists, case workers and administrators who care for older adults in their practice.

This event was presented by Summa Health Senior Health, a division of the Summa Health Complex Care Institute.

Continuing Education Credits

Summa Health System is accredited by the Ohio State Medical Association to provide continuing medical education for physicians. Other disciplines, including nursing, may participate in this CME activity since multiple credentialing bodies accept the AMA PRA Category 1 Credit™ as equivalent CE or CEU, as long as the topic is in the applicant’s field or discipline. Summa Health System designates this live activity for a maximum of 7 AMA PRA Category 1 Credits™.
