Referrals
Summa Health makes it easy to send referrals to our organization using these four options:
1. Epic to Epic
2. CareLink
3. By Fax
4. By Phone
Epic to Epic
Did you know that if you are a fellow Epic customer, you can make a referral to Summa Health directly from Epic? If you don’t know how to do this, please reach out to your organization’s Epic support team.
Carelink
For Community Providers and clinical staff that currently have CareLink access, you can send a referral directly through the CareLink portal.

Please note: The site agreement is not handled electronically. When we receive a request through the portal, we first obtain the necessary approvals. After approval, we email the user the Site Agreement as an attachment, requesting that the administrator or lead physician sign and return it to us.
Once we receive the signed agreement, we will set up the account using the Site Wizard. This process includes creating the provider group, user context, department name, assigning a specific username, temporary password, and other relevant details.
After the account is set up, we send an email to the user with instructions on how to access and set up their account, including their username and temporary password.
If you do not have a CareLink Account but would like to send a referral (with the required patient identifiers), you can request an account by clicking the button below.
Once we receive your request, we will email a Site Agreement to the address provided in the account request. Please sign and return the agreement via email.
After we receive the signed agreement, we will create your account within two business days and send instructions for submitting referrals. Referrals will be routed to our Referral Coordinators, who will schedule your patient. You will need to assign a site administrator to request additional accounts and reset passwords.
Once you have a Carelink account, making a referral to Summa Health will be a much more efficient process AND it will allow you to see the entire encounter.
