Community Health Workers

Summa Health recognizes the critical role that social drivers of health (SDOH) play in determining a person's overall health and well-being. SDOH are the non-medical factors that influence health outcomes. These conditions include:
  • The environment in which people are born, grow, work, live and age.
  • Economic and social conditions.
  • Factors such as political, socioeconomic and cultural circumstances.
  • Access to healthcare, education, safe housing, transportation and nutritious food.
 
Summa Health employs Community Health Workers (CHW) to collaborate with the healthcare team to ensure wrap around care. Our CHWs provide resources to patients outside of the healthcare facilities to improve health outcomes.
