This program offers a unique experience for the clinical pharmacist who desires advanced training at an integrated health care system focused on patient-centered and cost-effective symptom management. The resident will gain experience through interactions with multiple interdisciplinary teams of healthcare professionals devoted to providing optimal care to patients with life-limiting illnesses. Activities include daily rounds on the 12-bed Acute Palliative Care Unit and weekly involvement with the outpatient palliative care clinic and hospice teams serving an average daily census of over 150 patients along with a variety of other patient populations and specialties. The resident will participate in the educational activities and train alongside medical fellows completing an accredited Hospice and Palliative Medicine Fellowship Program.
